Title: Van Doolhof Naar Richting: Passende Zorg Begint met de Juiste Volgorde

Summary: Veel zorgorganisaties willen vooruit met technologie, maar lopen vast op implementatie, borging en opschaling. Innovaties starten hoopvol, maar blijven hangen in pilots, losse projecten of enthousiaste individuen. Niet door gebrek aan goede tools, maar doordat het kiezen, laten landen en volhouden complex blijkt. Hoe voorkom je dat technologie een extra laag wordt bovenop bestaande zorg, in plaats van een beweging die echt iets verandert? In deze sessie delen we hoe Zorggroep Solis een andere route koos. Niet starten bij technologie, maar bij de vraag: wat is hier écht nodig? Vanuit de Schijf van Vijf – een reflectiemodel voor passende zorg – wordt eerst gekeken naar wat iemand zelf kan, wat opnieuw geleerd kan worden en wat de omgeving kan betekenen. Technologie komt pas daarna in beeld. Met Floortje wordt deze stap praktisch ondersteund. Niet door meer innovaties toe te voegen, maar door zorgverleners te helpen om passende keuzes te maken in een wereld van overvloed. Deze aanpak maakt implementeren eenvoudiger, borging vanzelfsprekender en opschaling realistischer. Geen nieuwe oplossing, maar een andere volgorde. En precies daar begint duurzame verandering.
